The carer emergency card scheme is designed to provide you with peace of mind that your contingency plans be accessible to all should any unplanned emergency occur to you.

You will need to complete an entry on Cornwall’s Carer Register to access the online emergency carer card application form.

We require signatures from at least one nominated person who can provide care in your absence. We also require a signature from the person you care for, if they have the capacity to

Your emergency plan will be supported by the Family Information Service during 9am till 5pm. Outside of these hours; Cornwall Lifeline will make sure the plan is put into place when it is needed.

You will be issued with a sized card with a unique ID number and the emergency plan telephone numbers. When this unique ID number is quoted to either FIS or lifeline they will call contact the people you have requested in the case of an emergency.
